- First and last name must be the same as on the employee’s pay stub.
- Must use agency or affiliated organization email address. This address must belong to the user.
- The Organization Name field must contain the full agency name typed out (no abbreviations or misspellings).
- The Current VDH Employee field must be listed as “No”.
- The Race/Ethnicity field must be filled in from the drop down menu.
- If the user is a manager or supervisor the Manager/Supervisor box must be checked.
