Thank you for your interest in the VVFC program. We are excited to have you aboard! Together we can work to protect Virginia's children against vaccine preventable diseases.

Here is how to get started…

• Call the VVFC program at 1-800-568-1929 or 1-804-864-8055 to receive an Enrollment Packet or click on the links below to print an Enrollment Form.
  
• Complete the VVFC Enrollment Form. Keep a copy and mail the original to the VVFC office.
  
• It will take 5 business days to process your enrollment and assign your practice a VVFC PIN (Practice Identification Number). You will use your PIN to identify your practice when communicating with the VVFC office.
  
• Once your enrollment is processed, you will be contacted by a VVFC consultant and we will schedule an enrollment visit to introduce the program to you.
