Guidelines To Follow:

Returning or Transferring Vaccine:
Providers may phone, fax, or mail their vaccine returns and transfers to the VVFC office using the return form located on the forms page (or at the bottom of the page). Please indicate on the VVFC Vaccine Return form the reason for your vaccine return (i.e., expired, damaged, transfer). When transferring vaccine please indicate the facility to which you are transferring the vaccine.

Phone Orders:
Vaccine orders placed by phone will be accepted 8:30 a.m. to 4:00 p.m. Monday through Friday. If your practice decides to place a vaccine order by phone and is unable to get through to the VVFC Order Center, leave your name, PIN, medical facility's name, and phone number on the voicemail. A VVFC Order Center representative will contact you to place your vaccine order.

Ensuring Vaccine Delivery:
It is the responsibility of enrolled VVFC providers to notify VVFC if their address and business hours change to ensure appropriate vaccine delivery. If providers are closed during lunch hours or certain holidays, they should specify this information on their registration form. Holiday hours may alter the Order Center schedule as well as the days and times the VVFC office is open.

Accountability:
All VVFC Providers are required to submit reports to monitor vaccine use. Federally Qualified Health Centers (FQHCs) and large private practices must submit accountability reports on a monthly basis. Local Health Departments (LHDs) must submit quarterly inventory reports. All other providers submit benchmarking Tally Reports.
