Nursing Home Admissions & Medicaid Waivers

Roanoke City Health Department provides the screening assessment for several Medicaid waivers and prescreening for nursing home admissions.

Elderly and Disabled Services , ConsumerDirected Personal Attendant Services and HIV Waivers

These are Medicaid waivers that provide a scope of services to those individuals who are disabled or elderly. To receive these services the individual must be screened by the health department and a social worker from the Department of Social Services. Services may include

Individual & Family Developmental Disabilities Support Waiver

This is another Medicaid waiver for individuals age 6 and above with conditions related to mental retardation ( MR ), but who do not have a diagnosis of MR. It includes, but is not limited the diagnosis of cerebral palsy, epilepsy, autism, developmental delay or brain injury.

Services include:

Referrals must be made to DMAS in Richmond . A referral application may be obtained from the DMAS website or by calling (540) 983-1089. Once DMAS processes the referral it is sent to the health district where the individual lives. A comprehensive screen will be conducted to determine whether the individual qualifies for this waiver.

Barter Program

The Barter Program is a special agreement made with Salem Veterans Medical Center (VAMC) to provide skilled nursing services to veterans in the community in exchange for office space at the VAMC site. Home visits are made to these veterans to provide wound care, post -surgery assessment, disease education and follow up, or for environmental, functional or equipment needs for a chronically ill veterans. Only referrals from the VA may be received for this service.
