Nursing Home Admissions & Medicaid Waivers

Roanoke City Health Department provides the screening assessment for several Medicaid waivers and prescreening for nursing home admissions.

Elderly and Disabled Services , ConsumerDirected Technology Assisted and HIV Waivers

These are Medicaid waivers that provide a scope of services to those individuals who are disabled or elderly. To receive these services the individual must be screened by the health department and a social worker from the Department of Social Services. Services may include

• Assistance with daily care such as bathing, dressing toileting, and household chores
• Respite care for care giver support
• Adult day care
• Admission to a care facility

Individual & Family Developmental Disabilities Support Waiver

This is another Medicaid waiver for individuals age 6 and above with conditions related to mental retardation ( MR ), but who do not have a diagnosis of MR. It includes, but is not limited the diagnosis of cerebral palsy, epilepsy, autism, developmental delay or brain injury.

Services include:

• personal care
• companion care
• crisis stabilization
• environmental modifications
• family/caregiver training
• skilled nursing services
• respite care
• supported employment
• institutional care

A request for a screen may be obtained from the DMAS website - http://www.dmas.virginia.gov - or by calling (540) 857-7600 Ext. 325. Once DMAS processes the referral it is sent to the health district where the individual lives. A comprehensive screen will be conducted to determine whether the individual qualifies for this waiver.
