Roanoke Care Connection for Children

The Roanoke Care Connection Center serves children with special health care needs from 34 counties. We provide care coordination, medical insurance benefits evaluation and coordination, information and referral to community resources, and consultation with community providers on issues relating to special health care needs. Our role in providing care coordination is to assure that these families have assistance in guiding them though the confusing maze of services from health care plans, schools systems, state agencies and private agencies. It is often left to families to figure out who will provide for services such as physical, occupational, speech, and mental health therapies. Families many times are left un-served while agencies and programs try to decide about whom is responsible to pay for or provide these services. Our goal is to assist families in navigating though these systems.

We have a Pool of Funds to assist families who are uninsured or underinsured that have a gross income at or below 300 percent of the Federal Poverty Level. These funds help purchase medication, supplies or equipment that is not covered by insurance or for children without insurance. Eligibility for these funds is as follows:

Children have a special health care disorder that has a physical basis; Have lasted, or expected to last, at least 12 months and produce one or more of the following sequel.

Currently, we maintain some clinic activities to assist with specialty care in the area. These clinics include Urology, Neurology, Hearing Impairment, Rheumatology, Maxillofacial, and Cystic Fibrosis clinics. We always look for services that children can receive in their own communities. This is not always possible since some specialists are not available in their communities.

Included in our plans for children with special health care needs are to teach families and patients to advocate for themselves , to assist with transition to adult care, and to include the child in every aspect of his own care. Many children with special health care needs grow to be productive members of society and need to have skills for self-care and self-advocacy.

SIDS Care Coordination

Families who have experienced the death of an infant through sudden death syndrome may contact the care coordination nurse at (540) 983-1089. The nurse works to provide education and resources for grief management.
