Care Connection for Children (CCC)

What
CCC helps families of children with special health care needs coordinate medical care with community and educational resources.

For Whom
Children from birth through age 21 with physical disorders or special health care needs who need support not usually required by children of the same age.

FAMIS Plus or private insurance. Financial assistance may be available to families with income at or below the federal poverty level
