For information concerning vital records, please call your
local health department between the hours of 8:30 am and 5:00 pm,
Monday through Friday. You may find the telephone number for each
local health department in Chesterfield Health District on our Index
page.
Death
Certificates
A death certificate is filed with the local health department
in the city or county in which the death occurred. Certified
copies of death certificates are available upon request
to those persons eligible to receive them. Certified copies
are $12 per copy. Cash, money orders or checks are accepted.

Certificates
Certified copies of birth certificates
for births that occur in the State of Virginia must be obtained
from the Virginia Department of Health, Office
of Vital Records and Health Statistics, in Richmond, VA.
