Eligibility Information for Health Services

Eligibility requirements vary depending on the service provided. Some services are free, some are a flat fee and others are based on a sliding fee scale. Sliding fee scale means that in general, the more income you have, the larger the fee you pay for services. Financial eligibility is determined annually and whenever there is a change in financial status. You must have identification and proof of income when services are provided.


Health Services

Childhood Immunizations

Special school based immunization clinics are provided periodically.

Adult Immunizations

Childhood Lead Poisoning

Children under six years of age who are at risk of lead poisoning are referred to their primary care provider for lead testing. A health educator offers case management and educational services to families of children with elevated lead levels.

Comprehensive Health Investment Project (CHIP)

Provides case management and family support to assure access to health care, to help children become and stay healthier and promote family self-sufficiency. Provides medical case management, developmental screening and referral to community resources, transportation to medical appointments, teaching of parental skills and assisting with educational issues.

Sexually Transmitted Disease Services (STD)

HIV Services

For more information about HIV or other sexually transmitted diseases and to locate other anonymous testing sites in Virginia, call 1-800-533-4148. En Espanol 1-800-344-7432.

Medication assistance for protease inhibitors, antiretrovirals etc., is available based on eligibility criteria through the AIDS Drug Assistance Program (ADAP) administered by the Virginia Department of Health.

Family Planning

Provides physical examinations, counseling regarding methods of birth control, sexual responsibility, risk reduction and prevention for sexually transmitted diseases (STDs) and AIDS. Instruction on breast self exams. Birth control method of choice is provided. Persons under 18 may be eligible for free services.

Pregnancy Testing

Available by appointment. There is a charge for this service.

Maternity

Comprehensive prenatal care including periodic physical exams, lab testing, assessment of weight, blood pressure and nutritional counseling.

Nutrition

The "Loving Steps" Nutrition Program provides one on one counseling to pregnant women with the goals of reducing infant mortality and low weight births. Counseling takes place at the time of the prenatal visit in the OB/GYN private doctor's office.

Provides nutrition assessment, screening and follow-up counseling for maternity and family planning clients throughout the Crater Health District.

Resource Mothers Program

Provides guidance and assistance to pregnant teens until baby's first birthday. Supportive services include assisting teens with early entry into prenatal care, keeping medical appointments, encouraging healthy eating, developing parenting skills, improving communication skills between the teens and their families and partners and teaching preventive health behaviors. Program also assists with mentoring, transportation to appointments and assistance with obtaining help in the community.

Teen Pregnancy Prevention

Acts as fiscal agent for funds from Virginia Department of Health targeting programs to reduce teen pregnancy rates. Collaborates with community coalitions, agencies and groups to promote abstinence as a primary means of delayed parenthood.

WIC (Women, Infants and Children Program)

Supplemental food program for children from birth to age 5, pregnant and breast-feeding women. Services provided include nutritional counseling/instruction and supportive services for breast-feeding mothers.

Vital Records

Provides information and forms to utilize in securing birth certificates and establishments of paternity. Issues death certificates for all deaths occurring in the city/county. Death certificates cost $12 per certified copy.

Birth certificates can be obtained through the Office of Vital Records in Richmond. There is a $12 charge per copy.


Health Promotion/Community Outreach

Cardiovascular Risk Reduction Program

(Cardiovascular Coordinator-Linda Baker, RN) Phone: (804) 863-1652 ext 8954

Provides health screenings (including blood pressure and cholesterol) in the community or at worksites to help assess an individual's coronary risk status. Educational programs which focus on changing lifestyle behaviors are an important component of the program.

Mammography Screening Program

Provides free mammography screening for low-income, uninsured women.
