No Eligibility required for Free Services and Flat-Fee Services. Free and Flat-Fee Services are free to all minors under 17 years of age. These services include:
- Lead Test
- Sexually Transmitted Diseases ( free for all, minors & adults )
- Immunizations
- Pregnancy Test
- Tuberculosis

Service eligibility determination is based on family size, income, and
Fiscal Year 2006 Sliding Scale Family Size, Income Chart (valid thru June 30, 2007).

Other Business Services
Voter Registration
The health department is an official Voter Registration site. The Virginia Voter Registration Application form is available to:
- Register to vote
- Change your name or address, if changed since the last time you voted
- If requested, assist in completing form is provided
Forms are forwarded to the City of Portsmouth Registrar Office on a regular basis
Notary Services
Notaries at the health department act as an impartial witness during the execution of a legal or commercial transactions requiring a notary to witness, sign, and stamp with the notary seal (Fee: $2 per signature)
