Enrollment Packages/Forms

Below are the various types of enrollment forms to be filled out in order to gain access to VIIS. These forms are based on your organization type and needs.

Registration Instructions

Each organization needs to have at least one individual who will be the VIIS Administrator. This individual will be the main point of contact for VDH and will have full system access, including the ability to add/delete all additional users.  Each additional user will fill out User Registration forms and will be maintained by the assigned Administrator.

1. VIIS Administrator Registration Forms: Each administrator must complete and return to VDH for processing
2. VIIS User Registration Forms: Each user in addtition to the administrator must renew and complete these forms. Admins will maintain these forms.

Health Department Registration

Each organization needs to have at least one individual who will be the VIIS Administrator. This individual will be the main point of contact to the central office and will have the ability to add/delete VIIS users.  Each additional user will fill out the User Registration forms and maintained by the assigned Administrator.
