PATIENT SERVICES

Services provided include:

Flat Fee Services:

Free Services: (no proof of income required)

Free Service:  ( proof of family's gross income required)

Women, Infants, & Children (WIC) supplemental food program designed for pregnant, post-partum and breast-feeding mothers (up to 6 months), as well as their infants and children up to the age of 5 years, with food packages that help provide needed nutrition. The WIC program is a non-chargeable program.

Sliding Fee Services:

(Charges for the following services are on a sliding fee basis. The percentage you may be asked to pay is based on your family's gross income and your family size)

At the time of visit for a sliding fee service you may:

If you choose to be billed or pay only a portion of your bill that day and sign a Payment Plan.  You will need to pay something on your bill every 30 days from the date of your service and have your bill paid in full by the end of 90 days. Doing this prevents service charges and late fees from being added.

Keeping your bills paid makes it possible for us to continue providing services to you.

Health Department Staff providing these services include:

Physicians, dentists and dental assistants, nurse practitioner, registered nurses, epidemiologist, nutritionists and nutritional aides, environmental health specialist, clinical aides, and specialized clerical staff.
