Portsmouth Health Department maintains death certificates on deaths that occurred in the City of Portsmouth, Virginia only

Who can request a Death Certificate?
Mother/Father
Adult - Sister/Brother
Court Appointed Guardian
Husband/Wife
Adult Child
Funeral Directors and legal representative of family
Beneficiary named on a policy

Virginia Department of Health will not issue Vital Records certificates to the following:
Aunts/Uncles
Grandparents/Grandchildren
Friend
Fiancées
Other non-related persons

Also, Vital Records may be obtained online from the Vital Checks website

Paternity Affidavits
If the biological parents of a child are not married at the time of birth, in order for the father's name to appear on the child's birth certificate, an acknowledgement form must be completed. The Acknowledgement of Paternity Form ( VS22 ) is available at the health department. Both parents must complete and sign form in the presence of a notary public. The form is then mailed to the Division of Vital Records in Richmond, Virginia.
