Critical Access Hospital Facts

An important federal and state initiative for rural areas is the Medicare Rural Hospital Flexibility Program, also known as the Flex program or the Critical Access Hospital program. Recognizing the struggle of financially pressed rural hospitals, Congress appropriated funds in the Balanced Budget Act of 1997 for states to assist small rural hospitals to convert to "Critical Access Hospitals" (CAH). This status allows the hospital to receive cost-based reimbursement from Medicare. Critical Access Hospitals must meet the following federal criteria:

  • A limit of 15 acute care beds and 10 swing beds
  • A length of stay averaging no more than 96 hours
  • A daily census of 15 acute care patients
  • 24-hour emergency services
  • Agreements with a network hospital for patient referral and transfer
