CV Requirements

Your CV should not exceed 3 pages in length. (Please note that practice sites prefer a short CV for initial review). Your CV should include your name & page number on each page. The following information MUST be included:

• Personal Information
• Date of Availability
• Education, including residency program(s)
• Employment History
• Licensure/Certification (with State(s) and License Number)

It is highly recommended that the following information be included:

• Language capabilities (some sites are looking for multilingual candidates)
• Specialty skills, such as management, reimbursement, etc.

The following items are optional:

• Honors and Achievements
• Research/Publications
• Professional Organizations
• Volunteer Experience
