Print out a copy of the HIV/ STD Literature Request Form. Fill the date, your name, phone number and physical address. Next, fill in the quantity desired next to the publication(s) you want to order. HD16 POSITIVE LIVING MANUAL AND TRACKER
This manual is intended for HIV positive persons. It provides a wide variety of information on living with HIV including, but not limited to, nutrition, finding a medical provider, stress relief, safer sex, treatment options, legal issues and statewide resources. The tracker is included to assist with medication adherence as well as charting lab results and documenting symptoms.
