Death Certificates, Vital Records

Birth Certificate Applications and Death Certificates

Death records are filed at the health department in the county or city in which the death occurred, not the county or city in which the person lived. Certified copies can be obtained only by a relative of the deceased or by a representative of a funeral home. The cost per copy of a death record is $12.00. The Three Rivers Health District has death records on file from 2003 to the present for deaths occurring in Essex, Gloucester, King & Queen, King William, Lancaster, Mathews, Middlesex, Northumberland, Richmond, and Westmoreland counties. Copies of death records prior to 2003 and all birth records are filed in Richmond and can be obtained by sending the completed request form to the following address:
