Internal Reporting: Senior management of the Health Department supports the internal reporting of suspected fraud, wrong doing, or policy violations directly to the Internal Audit Director (at 804-864-7450, or at email address: This often allows an investigation to begin sooner, and reduces reporting requirements. In any cases with proven material financial loss or criminal wrongdoing involving state employees, Internal Audit is required to report the findings to the Office of the Inspector General and the Auditor of Public Accounts.

To report such items, please include in an e-mail the following:

  • The description of alleged event or policy violation.
  • The date and location of the event or violation.
  • Identity of persons involved in alleged event or violation.
  • Names of any witnesses to events or violations.
  • Optional - name and method to contact person reporting event or violation, to obtain more details.
