You will receive an Outcome report indicating:\n<ul style=\"list-style-type: circle;\">\n<li>Participant date of birth<\/li>\n<li>Contact date (if reached)<\/li>\n<li>Planned quit date (if accepted services)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>It is our goal to provide technical assistance needed to include referral service into your practice.
