SUMMARY:Person-Centered Practices Knowledge\, Skills\, and Abilities for Community Care Hubs
DESCRIPTION:Person-centered practices help people live fully in their communities and work toward their own life goals. Supports should be designed around what each person wants and needs. Community care hubs (CCHs) are well-positioned to deliver this kind of support. They bring together community-based service providers and health care organizations to address factors that affect health and well-being. Frontline staff play a central role. They assess each person’s needs\, connect them to services and resources\, and create person-centered plans that reflect individual goals and desired outcomes. \nHosted by ACL’s No Wrong Door Resource Center\, this webinar will highlight two new resources that define the essential knowledge\, skills\, and abilities frontline community-based service providers need to effectively deliver person-centered support. The session will highlight Southern Alabama Regional Council on Aging’s Community Care Solutions and the promising practices this CCH uses to train the community workforce. \nThis webinar will be recorded\, and the recording link and materials will be made available to registrants afterward.
