Chesterfield Health District will host vaccine clinics at the following locations. The student must be enrolled in that school to receive the vaccine(s) on the assigned date. <\/strong>Students are not eligible at other schools in which they are not enrolled.<\/p>\n

    \n
  1. Parents can check their student's records<\/a> before enrolling them.<\/li>\n
  2. Read the vaccine information sheets at the links under each vaccine below:\n
      \n
    1. In the middle schools we will administer the Tdap<\/a>, Meningococcal ACWY<\/a>, and HPV<\/a> vaccines.
      \n(      Vacuna Tdap<\/a>, meningoc\u00f3cica ACWY<\/a>, Vacuna contra el VPH<\/a>)<\/li>\n
    2. In the high schools we will administer the meningococcal<\/a> ACWY vaccine. (meningoc\u00f3cica ACWY)<\/a><\/li>\n
    3. If a student should need other immunizations, we will share additional resources.<\/li>\n<\/ol>\n<\/li>\n
    4. Parents should register their students online (at the link for each school below) to receive these immunizations.
      \n\u2666 Enter the student's insurance information if the student is covered by insurance. Upload a photo<\/strong> of the front and back of the card where indicated.
      \n\u2666 Check the boxes<\/strong> for all immunizations that are needed.
      \n\u2666 If you have a problem with registration, call 804-748-1750 for assistance.
      \n\u2666 Please register ahead of time for these clinics to allow us time to evaluate the records and insurance. Internet access in this area is not reliable so we need as much notice as possible.<\/li>\n

      1. Providence Middle: register by Thursday, April 30<\/li>\n
      2. L.C. Bird High: register by Friday, May 8<\/li>\n
      3. Manchester Middle: register by Sunday, May 10<\/li>\n
      4. Falling Creek Middle: register by Monday, May 11<\/li>\n
      5. Carver Middle: register by Thursday, May 14<\/li>\n
      6. Carver College and Career Academy: register by Thursday, May 14<\/li>\n
      7. Meadowbrook High: register by Thursday, May 14<\/li>\n
      8. Matoaca High: register by Thursday, May 14<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n
