{"id":1352,"date":"2022-10-06T17:15:12","date_gmt":"2022-10-06T17:15:12","guid":{"rendered":"https:\/\/www.vdh.virginia.gov\/clinicians\/?page_id=1352"},"modified":"2022-10-06T17:15:38","modified_gmt":"2022-10-06T17:15:38","slug":"covid-19-and-monkeypox-updates-for-virginia-2","status":"publish","type":"page","link":"https:\/\/www.vdh.virginia.gov\/clinicians\/covid-19-and-monkeypox-updates-for-virginia-2\/","title":{"rendered":"COVID-19 and Monkeypox Updates for Virginia"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1137\" src=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/134\/2022\/04\/Letterhead-Greene_2022-1.png\" alt=\"\" width=\"1700\" height=\"452\" \/><\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center\"><b>COVID-19 and Monkeypox Updates for Virginia<\/b><\/p>\n<p style=\"text-align: center\"><span style=\"font-weight: 400\">October 6, 2022<\/span><\/p>\n<p><span style=\"font-weight: 400\">Dear Colleague:<\/span><\/p>\n<p><span style=\"font-weight: 400\">I am writing to provide you with brief updates on COVID-19 and monkeypox.<\/span><\/p>\n<p><b>VDH Adopts CDC\u2019s Revised COVID-19 Infection Prevention and Control Guidance<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">On September 23, 2022, the Centers for Disease Control and Prevention (CDC) released <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/infection-control-recommendations.html\"><span style=\"font-weight: 400\">updated COVID-19 infection prevention and control (IPC) recommendations<\/span><\/a><span style=\"font-weight: 400\"> for healthcare personnel (HCP). Corresponding guidance for <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/guidance-risk-assesment-hcp.html\"><span style=\"font-weight: 400\">isolation and work restriction of HCP<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/hcp\/mitigating-staff-shortages.html\"><span style=\"font-weight: 400\">strategies for mitigating staff shortages<\/span><\/a><span style=\"font-weight: 400\"> was also updated.\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Healthcare facilities are encouraged to review the recommendations in detail and make updates to their policies and procedures as needed.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><a href=\"https:\/\/covid.cdc.gov\/covid-data-tracker\/#county-view?list_select_state=all_states&amp;list_select_county=all_counties&amp;data-type=Risk\"><span style=\"font-weight: 400\">Community Transmission<\/span><\/a><span style=\"font-weight: 400\"> is currently the metric used to guide select IPC measures in healthcare settings. Healthcare facilities should review the Community Transmission levels weekly to determine if modifications to current IPC measures are needed.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">When SARS-CoV-2 community transmission levels are high, source control (wearing a well-fitting facemask, cloth mask, or respirator) is recommended for everyone in a healthcare setting where patients could be encountered.\u00a0 If community transmission is not high, healthcare facilities may opt not to recommend universal source control.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Source control is also recommended for people in healthcare settings who have suspected or confirmed SARS-CoV-2 infection or another respiratory infection, had close contact (patients and visitors) or a higher-risk exposure (HCP) with someone with COVID-19, for 10 days after exposure, or who reside or work in an area of the facility with a COVID-19 outbreak.\u00a0\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Asymptomatic patients with close contact with someone with COVID-19 should have a series of three viral tests for SARS-CoV-2. Testing is recommended to be done promptly, but no earlier than 24 hours after the exposure.\u00a0 If the first test is negative, a second test should be done 48 hours later and, if negative, a third test should be done 48 hours later.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Testing is not generally recommended for asymptomatic people who are recovering from COVID-19 in the previous 30 days.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">For those who have recovered in the prior 31\u201390 days, testing with a rapid antigen test, not a molecular test, can be considered.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">COVID-19 vaccination status is no longer used to inform source control, screening testing, or post-exposure recommendations.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Performance of expanded screening testing of asymptomatic HCP with known COVID-19 exposures is at the discretion of the facility.<\/span><\/li>\n<\/ul>\n<p><b>Monkeypox Updates\u00a0<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">On September 29, 2022, CDC issued a <\/span><a href=\"https:\/\/emergency.cdc.gov\/han\/2022\/han00475.asp\"><span style=\"font-weight: 400\">health advisory<\/span><\/a><span style=\"font-weight: 400\"> about severe monkeypox disease among immunocompromised people.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">CDC offers clinical consultation (email <\/span><a href=\"mailto:eocevent482@cdc.gov\"><span style=\"font-weight: 400\">eocevent482@cdc.gov<\/span><\/a> <span style=\"font-weight: 400\">or call the CDC Emergency Operations Center at 770-488-7100).<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">On September 28, 2022, CDC revised its <\/span><a href=\"https:\/\/www.cdc.gov\/poxvirus\/monkeypox\/health-departments\/vaccine-considerations.html\"><span style=\"font-weight: 400\">Interim Clinical Considerations for Use of JYNNEOS and ACAM2000 Vaccines during the 2022 U.S. Monkeypox Outbreak<\/span><\/a><span style=\"font-weight: 400\"> and other supporting vaccine websites and resources.\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">On September 26, 2022, <\/span><a href=\"https:\/\/www.vdh.virginia.gov\/monkeypox\/healthcare-providers\/vaccine-guidance\/\"><span style=\"font-weight: 400\">VDH expanded eligibility of monkeypox expanded post-exposure prophylaxis<\/span><\/a><span style=\"font-weight: 400\"> to include (1) Any person, of any sexual orientation or gender, who is living with HIV\/AIDS; or (2) Any person, of any sexual orientation or gender, diagnosed with any sexually transmitted infection in the past three months.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">VDH developed <\/span><a href=\"https:\/\/www.vdh.virginia.gov\/monkeypox\/healthcare-providers\/\"><span style=\"font-weight: 400\">five new video training modules<\/span><\/a><span style=\"font-weight: 400\"> for providers about monkeypox (overview, prevention\/containment, testing, vaccination, treatment\/patient management).<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">VDH is expanding its network of providers who can order JYNNEOS vaccines. The intent is to position this vaccine with providers who treat populations at high risk for monkeypox.
