{"id":6,"date":"2016-01-26T16:23:43","date_gmt":"2016-01-26T16:23:43","guid":{"rendered":"https:\/\/www.vdh.virginia.gov\/communications\/?page_id=6"},"modified":"2024-08-21T19:45:36","modified_gmt":"2024-08-21T19:45:36","slug":"advance-health-care-directive-registry","status":"publish","type":"page","link":"https:\/\/www.vdh.virginia.gov\/commissioner\/administration\/advance-health-care-directive-registry\/","title":{"rendered":"Advance Health Care Directive Registry"},"content":{"rendered":"<div id=\"pl-6\"  class=\"panel-layout\" ><div id=\"pg-6-0\"  class=\"panel-grid panel-no-style\" ><div id=\"pgc-6-0-0\"  class=\"panel-grid-cell\" ><div id=\"panel-6-0-0-0\" class=\"so-panel widget widget_sow-editor panel-first-child panel-last-child\" data-index=\"0\" ><div class=\"panel-widget-style panel-widget-style-for-6-0-0-0\" ><div\n\t\t\t\n\t\t\tclass=\"so-widget-sow-editor so-widget-sow-editor-base\"\n\t\t\t\n\t\t>\n<div class=\"siteorigin-widget-tinymce textwidget\">\n\t<ul>\n<li><a href=\"https:\/\/vhi.org\/ConnectVirginia\/adr.asp\">Advance Healthcare Directives Registry<\/a><\/li>\n<\/ul>\n<\/div>\n<\/div><\/div><\/div><\/div><\/div><div id=\"pg-6-1\"  class=\"panel-grid panel-no-style\" ><div id=\"pgc-6-1-0\"  class=\"panel-grid-cell\" ><div id=\"panel-6-1-0-0\" class=\"so-panel widget widget_sow-editor panel-first-child panel-last-child\" data-index=\"1\" ><div\n\t\t\t\n\t\t\tclass=\"so-widget-sow-editor so-widget-sow-editor-base\"\n\t\t\t\n\t\t>\n<div class=\"siteorigin-widget-tinymce textwidget\">\n\t<p>A person may submit any of the following documents and the revocations of these documents to the Department of Health for filing in the Advance Health Care Planning Registry established pursuant to Code of Virginia Article 9 (\u00a7 54.1-2995):<\/p>\n<ul style=\"padding-bottom:2em;\">\n<li>A health care power of attorney.<\/li>\n<li>An advance directive created pursuant to Article 8 (\u00a7 <a title=\"https:\/\/law.lis.virginia.gov\/vacode\/54.1-2981\/\" href=\"https:\/\/law.lis.virginia.gov\/vacode\/54.1-2981\/\" target=\"_blank\" rel=\"noreferrer noopener\">54.1-2981<\/a> et seq.) or a subsequent act of the General Assembly.<\/li>\n<li>A declaration of an anatomical gift made pursuant to the Revised Uniform Anatomical Gift Act (\u00a7 <a title=\"https:\/\/law.lis.virginia.gov\/vacode\/32.1-291.1\/\" href=\"https:\/\/law.lis.virginia.gov\/vacode\/32.1-291.1\/\" target=\"_blank\" rel=\"noreferrer noopener\">32.1-291.1<\/a> et seq.).<\/li>\n<li>Any other document that supports advance health care planning, including Durable Do Not Resuscitate Order or portable medical order forms.<\/li>\n<li>The document may be submitted for filing only by the person who executed the document or his legal representative or designee.<\/li>\n<li>All data and information contained in the registry shall remain confidential and shall be exempt from the provisions of the Virginia Freedom of Information Act (\u00a7 <a title=\"https:\/\/law.lis.virginia.gov\/vacode\/2.2-3700\/\" href=\"https:\/\/law.lis.virginia.gov\/vacode\/2.2-3700\/\" target=\"_blank\" rel=\"noreferrer noopener\">2.2-3700<\/a> et seq.).<\/li>\n<\/ul>\n<p>\u00a7 54.1-2981.
