Include DOB, mom\u2019s last name, and birth hospital if known.<\/p>\n<h2><strong>Parents and Legal Guardians<\/strong><\/h2>\n<p>You can request a copy of your child\u2019s newborn screen from pediatrician or hospital.<\/p>\n<p>If you still need assistance, <a href=\"https:\/\/dgs.virginia.gov\/division-of-consolidated-laboratory-services\/lab-testing\/nbs\/\" target=\"_blank\" rel=\"noopener\">DCLS<\/a> can provide copies of patient reports to citizens as required by The Centers for Medicare and Medicaid Services (CMS). You can request copies of newborn screening reports for babies born in Virginia on or after March 1, 2001. Newborn screening records are not available for babies born before that date.<\/p>\n<h3>Release Form and Instructions<\/h3>\n<p>Please print the release form and submit the request to DCLS to receive a copy of your test results report.
