Newborn Screening kits may only be ordered by hospitals, midwives and physicians' offices. They may not be ordered by private citizens. Kit requests must be paid for in advance. The completed order form with payment must be submitted to the Cashier's Office, Department of General Services, for processing prior to the kits being shipped by DCLS.
