Mini-Grant Program\t\t\t\t\t\t<\/h1>\n\t\t\t\t\t\t<\/div>\n<\/div><\/div><\/div><\/div><div id=\"pg-4804-2\"  class=\"panel-grid panel-no-style\" ><div id=\"pgc-4804-2-0\"  class=\"panel-grid-cell\" ><div id=\"panel-4804-2-0-0\" class=\"so-panel widget widget_sow-editor panel-first-child panel-last-child\" data-index=\"3\" ><div\n\t\t\t\n\t\t\tclass=\"so-widget-sow-editor so-widget-sow-editor-base\"\n\t\t\t\n\t\t>\n<div class=\"siteorigin-widget-tinymce textwidget\">\n\t<h3>About the Program<\/h3>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The purpose of the Rural Health Partner Mobilization Mini-Grants are to provide funding to support a variety of projects that align with any of the four strategies prioritized by the CDC COVID-19 Rural Health Disparities Grant<\/span><i><span style=\"font-weight: 400\"> (listed below)<\/span><\/i><span style=\"font-weight: 400\">.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Projects within this grant have the flexibility to address secondary and tertiary effects of COVID-19, as well as other related health disparities within rural and\/or Medically Underserved Area\/population.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Priority\u00a0will be given to applicants that demonstrate a need for the funding, have a clear plan in place for using the funding, and can articulate the anticipated impact of the funding if received.<\/span><\/li>\n<\/ul>\n<h3>Priority Strategies<\/h3>\n<p><span style=\"font-weight: 400\">Four overarching program strategies aim to build infrastructures that both address disparities in the current COVID-19 pandemic and set the foundation to address future responses.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><i><span style=\"font-weight: 400\">Strategy 1: <\/span><\/i><span style=\"font-weight: 400\">Expand existing and\/or develop new mitigation and prevention resources and services to reduce COVID-19 related disparities among populations at higher risk and that are underserved<\/span><\/li>\n<li style=\"font-weight: 400\"><i><span style=\"font-weight: 400\">Strategy 2: <\/span><\/i><span style=\"font-weight: 400\">Increase\/improve data collection and reporting for populations experiencing a disproportionate burden of COVID-19 infection, severe illness, and death to guide the response to the COVID-19 pandemic<\/span><\/li>\n<li style=\"font-weight: 400\"><i><span style=\"font-weight: 400\">Strategy 3: <\/span><\/i><span style=\"font-weight: 400\">Build, leverage, and expand infrastructure support for COVID-19 prevention and control among populations that are at higher risk and underserved<\/span><\/li>\n<li style=\"font-weight: 400\"><i style=\"background-color: transparent\">Strategy 4: <\/i><span style=\"font-weight: 400\">Mobilize partners and collaborators to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved<\/span><\/li>\n<\/ul>\n<h3>Mobilizing Partnerships<\/h3>\n<ol>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Local Health Districts (LHDs) should engage with local Community-based Organizations (CBOs) who might be interested in pursuing this opportunity.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Interested CBOs should submit an application packet to their respective LHD.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The LHD refers the CBO to VA-SORH.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">VA-SORH facilitates the transfer of funds to reduce the administrative burden on LHD staff.<\/span><\/li>\n<\/ol>\n<h3><strong>Application\u00a0<\/strong><\/h3>\n<p><span style=\"font-weight: 400\">The application packet <\/span><i><span style=\"font-weight: 400\">(5 page limit)<\/span><\/i><span style=\"font-weight: 400\"> should consist of the following:<\/span><\/p>\n<ul>\n<li><b>Proposal Narrative<\/b>\n<ul>\n<li><span style=\"font-weight: 400\">Statement of Need<\/span><\/li>\n<li><span style=\"font-weight: 400\">Commitment to utilize grant funds to serve rural and\/or medically underserved communities within the established project period<\/span><\/li>\n<li><span style=\"font-weight: 400\">Demonstrate relevance to one or more of the four priority strategies listed above.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li><b>Population to be Served<\/b>\n<ul>\n<li><span style=\"font-weight: 400\">Demonstrate need for this program in the target community\/area<\/span><\/li>\n<li><span style=\"font-weight: 400\">Potential impact of the program<\/span><\/li>\n<li><span style=\"font-weight: 400\">Describe potential barriers or challenges<\/span>\n<ul>\n<li><span style=\"font-weight: 400\">Propose mitigation strategies, if applicable<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li><b>Project Timeline\/Work Plan\u00a0<\/b>\n<ul>\n<li><i><span style=\"font-weight: 400\">Work Plan Template and Example available upon request<\/span><\/i><\/li>\n<li><span style=\"font-weight: 400\">Project goals and objectives\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\">Metric Tracking Plan<\/span>\n<ul>\n<li><span style=\"font-weight: 400\">Quantifiable process or outcome measure<\/span><\/li>\n<li><span style=\"font-weight: 400\">Subrecipients are expected to report progress on proposed metrics on quarterly basis via REDCap Survey<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li><b>Categorized Budget &amp; Budget Narrative<\/b>\n<ul>\n<li><i><span style=\"font-weight: 400\">Budget Template and Example available upon request<\/span><\/i><\/li>\n<li><span style=\"font-weight: 400\">Clear and concise description of costs in each budget category<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3>Application, Reimbursement &amp; Project Period Timeline<\/h3>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Interested organizations <\/span><span style=\"font-weight: 400\">should submit an application to their respective LHD <\/span><b>on or before August 4th<\/b><span style=\"font-weight: 400\">.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The LHD should communicate the referral to the VA-SORH <\/span><b>on or before August 5th<\/b><span style=\"font-weight: 400\">.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">LHDs should submit completed application(s) and a <\/span><a href=\"https:\/\/docs.google.com\/document\/d\/10nyc71Hzex_Ey9eqZCP-Sj_iPRjwr17Z\/edit?usp=sharing&amp;ouid=104852581514222114213&amp;rtpof=true&amp;sd=true\"><span style=\"font-weight: 400\">Referral and Reallocation Agreement<\/span><\/a>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Brandon Rivenbark at\u00a0<\/span><a possible.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Funds will be reimbursed to CBOs by the VA-SORH on a <\/span><i><span style=\"font-weight: 400\">reimbursement by deliverable <\/span><\/i><span style=\"font-weight: 400\">basis.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">CBOs can submit invoices for reimbursement as needed.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The proposed project must be completed by <\/span><b>May 31<\/b><b>st<\/b><b>, 2023<\/b><span style=\"font-weight: 400\">.<\/span><\/li>\n<\/ul>\n<h3>Eligibility<\/h3>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Applicants should represent a public agency, tribal organization or private non-profit holding a 501(c)(3) tax-exemption in the Commonwealth of Virginia that is a designated rural service provider, Rural Health Clinic, Health Professional Shortage Area or a Medically Underserved Area\/Population.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The organization must demonstrate past or current programs\/projects that address the needs of the community or of a specific population.
