These may include:<\/p>\n<ul>\n<li>sickle cell disease<\/li>\n<li>epilepsy<\/li>\n<li>anxiety<\/li>\n<li>autism<\/li>\n<li>learning disorders<\/li>\n<\/ul>\n<p>CYSHCN may require specialized health and educational services to thrive. If you have a child with special needs, there are programs and resources that can help. VDH has several programs to assist:<\/p>\n<h4><a href=\"https:\/\/www.vdh.virginia.gov\/care-connection-for-children\/\" target=\"_blank\" rel=\"noopener\">Care Connection for Children (CCC):<\/a><\/h4>\n<ul>\n<li>Serves children from birth to age 21 with physical conditions that are expected to last a year or more.<\/li>\n<li>Provides care coordination or case management. This may include: referral and connection to needed clinical services, assistance in obtaining durable medical equipment and medications, and assistance in obtaining and understanding health insurance.<\/li>\n<\/ul>\n<h4><a href=\"https:\/\/www.vdh.virginia.gov\/child-development-services\/\" target=\"_blank\" rel=\"noopener\">Child Development Centers (CDC):<\/a><\/h4>\n<ul>\n<li>Assess children suspected of developmental delays, mental health concerns, behavior concerns, learning problems, and more.<\/li>\n<li>Core services include diagnostic assessment, care planning, follow-up care coordination and referral.<\/li>\n<\/ul>\n<h4><a href=\"https:\/\/www.vdh.virginia.gov\/sickle-cell-programs\/\" target=\"_blank\" rel=\"noopener\">Pediatric and Adult Comprehensive Sickle Cell Programs:<\/a><\/h4>\n<ul>\n<li>Serve children and people of all ages needing care for sickle cell disease.<\/li>\n<li>The pediatric centers are valuable partners with VDH for entry into care after children have been confirmed via the newborn screening bloodspot test.<\/li>\n<li>The adult program works hand in hand with the pediatric program to transition youth from pediatric care to adult care.<\/li>\n<\/ul>\n<h4><a href=\"https:\/\/www.vdh.virginia.gov\/bleeding-disorders-program\/\" target=\"_blank\" rel=\"noopener\">Virginia Bleeding Disorders Program (VBDP):<\/a><\/h4>\n<ul>\n<li>Serves children and adults with varying bleeding disorders. Disorders may include hemophilia and Von Willebrand Disease.<\/li>\n<li>Services include health insurance premium assistance, health insurance consultation, and pool of funds to obtain medications.<\/li>\n<\/ul>\n<h4><a href=\"https:\/\/www.vdh.virginia.gov\/vacares\/\" target=\"_blank\" rel=\"noopener\">Virginia Congenital Anomalies Reporting and Education System (VaCARES)<\/a><\/h4>\n<ul>\n<li>Serves as Virginia\u2019s mandated birth defects surveillance reporting system (<a href=\"https:\/\/law.lis.virginia.gov\/vacodefull\/title32.1\/chapter2\/article8.1\/\" target=\"_blank\" rel=\"noopener\">\u00a7 32.1-69.1<\/a>).<\/li>\n<li>Helps improve the knowledge, prevention, and outcomes of those affected by birth defects in Virginia.<\/li>\n<\/ul>\n<\/div>\n<\/div><\/div><\/div><\/div><\/div>","protected":false},"excerpt":{"rendered":"<p>Children and Youth with Special Health Care Needs (CYSHCN) have chronic physical, developmental, behavioral, or emotional conditions.
