Maternal Mortality Review<\/a> | Case Review Process<\/a> | Reports<\/a> | To Learn More<\/a><\/p>\n

Case Identification:\"teamworld\"<\/h2>\n
    \n
  1. Through the International Classification of Diseases (ICD), Tenth Revision, specifically those deaths identified with a cause code in the \u201cpregnancy, childbirth and the puerperium\u201d categories.<\/li>\n
  2. By matching birth or fetal death certificate with maternal death certificate information, regardless of cause of death.<\/li>\n
  3. By selecting cases where a Commonwealth of Virginia death certificate indicates the decedent was pregnant within three months of the death.\u00a0 This is a check box on Virginia\u2019s death certificate.<\/li>\n<\/ol>\n

    Records Collected:\"medicalrecords\"<\/h2>\n
      \n
    • Vital statistics (death certificates, birth certificates, fetal death records)<\/li>\n
    • Prenatal records<\/li>\n
    • Hospital records (outpatient and inpatient stays)<\/li>\n
    • Other provider\/specialist records: these records may be from preconception\/family planning clinics, or primary care providers, etc<\/li>\n
    • Autopsy reports and case findings from hospital, coroner, or other medical examiner<\/li>\n
    • Police\/investigative reports<\/li>\n
    • Social services reports<\/li>\n
    • Mental health records<\/li>\n
    • Substance abuse treatment records<\/li>\n
    • Medical transport records<\/li>\n
    • Court records<\/li>\n
    • News articles, where relevant<\/li>\n<\/ul>\n

      Team Review:\"team-gold\"<\/h2>\n

      The Team reviews de-identified case summaries.\u00a0 Team discussion and deliberation are governed by these values:<\/p>\n

        \n
      • Multidisciplinary review<\/li>\n
      • A public health approach<\/li>\n
      • Retrospective review<\/li>\n
      • Consensus decision-making<\/li>\n<\/ul>\n

        The Team collaboratively completes a Contributors to Mortality form.\u00a0 This discussion includes the Team\u2019s decision about the following primary dimensions of maternal deaths:<\/p>\n

          \n
        • the underlying cause of death;<\/li>\n
        • the preventability of the maternal death, which is broadly defined as a death that may have been averted by one or more changes in clinical care, facility infrastructure, community and\/or systems response to patient factors;<\/li>\n
        • the degree to which the death was pregnancy related;<\/li>\n
        • factors that potentially contributed to the death;<\/li>\n
        • recommendations for prevention and intervention.<\/li>\n<\/ul>\n
