The text\/phone call will contain a 6-digit verification code. If you do not have access to the phone number that is associated with your vaccination record, please call 877-VAX-IN-VA (877-829-4682) Monday through Friday 8 a.m. to 6 p.m.<\/span><\/p>\n<p style=\"padding-left: 40px\"><span style=\"font-weight: 400\">Choose your preferred method of contact and hit the respective button to initiate the message.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-2115 aligncenter\" src=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax2.png\" alt=\"\" width=\"397\" height=\"444\" srcset=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax2.png 544w, https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax2-268x300.png 268w\" sizes=\"auto, (max-width: 397px) 100vw, 397px\" \/><\/p>\n<p style=\"padding-left: 40px\"><b>Step 4:<\/b> <span style=\"font-weight: 400\">Once you have received your 6-digit verification code, enter it into the box and hit the \u201c<strong>Verify Code<\/strong>\u201d button.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p style=\"padding-left: 40px\"><b>Step 5: <\/b><span style=\"font-weight: 400\">Successful completion of the previous steps will present a new page containing your COVID-19 Vaccination Record.\u00a0<\/span><\/p>\n<p style=\"padding-left: 40px\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-2116 aligncenter\" src=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax3.png\" alt=\"\" width=\"393\" height=\"526\" srcset=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax3.png 532w, https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/17\/2021\/11\/vax3-224x300.png 224w\" sizes=\"auto, (max-width: 393px) 100vw, 393px\" \/><\/p>\n<p>&nbsp;<\/p>\n<p><b>What if the COVID-19 Vaccination Record with QR Code is not found?<\/b><\/p>\n<p style=\"padding-left: 40px\"><span style=\"font-weight: 400\">Vaccine providers in Virginia submit vaccination records to the Virginia Immunization Information System (VIIS), but they may have provided information that&#8217;s incomplete, out of date, or incorrect. <\/span><i><span style=\"font-weight: 400\">So, it&#8217;s likely VIIS has your record, but not the correct information.<\/span><\/i><\/p>\n<p style=\"padding-left: 40px\"><span style=\"font-weight: 400\">For instance:<\/span><\/p>\n<ul>\n<li style=\"list-style-type: none\">\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Mobile phone number does not match<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Name is spelled differently<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Address is out of date or incorrect<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 40px\"><span style=\"font-weight: 400\">To find the record, you should try re-entering your information without a ZIP code. \u00a0If your record still isn\u2019t found, you can contact the pharmacy\/doctor\/nurse who administered your COVID-19 vaccinations and ask them to update your vaccination record. You can also call 877-VAX-IN-VA (877-829-4682) for help; assistance is available Monday through Friday from 8 a.m. to 6 p.m. ET.<\/span><\/p>\n<p style=\"padding-left: 40px\">Vaccinations administered outside Virginia to Virginia residents may not be in VIIS.
