Your Health Your Voice – Help Shape Our Community’s Health!

We want to hear from you!

Help improve health in our community by sharing your thoughts. Please take our Community Health Assessment (CHA) Survey to tell us about health needs, challenges, and resources in our area. The survey is anonymous, should only take 5-10 minutes with an option to enter a $25 gift card raffle

Your input matters, especially for those facing big health challenges. Results will help the Community Healthcare Coalition of Greater Prince William, community organizations, and government agencies improve health services and plan programs that make our community healthier for everyone.
