The anonymous survey only takes 5-10 minutes to complete and is available in both <\/span><span style=\"font-family: arial, helvetica, sans-serif;\"><span style=\"text-decoration: underline;\"><a style=\"text-decoration: underline;\" href=\"https:\/\/bit.ly\/cha-survey\">English<\/a> <\/span><\/span><span style=\"font-family: arial, helvetica, sans-serif; color: #000000;\">and <\/span><span style=\"font-family: arial, helvetica, sans-serif;\"><span style=\"text-decoration: underline;\"><a style=\"text-decoration: underline;\" href=\"https:\/\/bit.ly\/cha-survey\">Spanish<\/a><\/span><\/span><span style=\"font-family: arial, helvetica, sans-serif; color: #000000;\">. PWHD wants to ensure that your voices are heard!<\/span><\/p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; color: #000000;\">\u00a0\u201cEvery response to the survey brings us closer to understanding what health challenges are impacting our residents and what truly matters to the people we serve,\u201d said Luke Davies, MPH, Health Director for PWHD. \u201cThis survey is more than just a tool; it\u2019s a way for us to listen, learn, and lead with your priorities in mind.\u201d<\/span><\/p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; color: #000000;\">This survey is part of a comprehensive assessment where information is collected about health problems impacting community members, what services are available to address them, and what needs to improve.
