To do that, we worked with community partners to find out what health issues are most important. This is called a Community Health Needs Assessment (CHA). We asked community members to share their thoughts through surveys and listening sessions. Thanks to everyone&#8217;s help, we finished the first part of the assessment. Now, we will use what we learned to make a plan to improve health in our community.
