This week, we continue highlighting some of the newest\u00a0Health Equity Fund<\/a>\u00a0(HEF) recipients. The HEF\u2014which is administered by the\u00a0Richmond and Henrico Public Health Foundation<\/a>\u00a0and supported by the City of Richmond and RHHD\u2014announced its most recent round of funding<\/a>\u00a0at the end of January.<\/p>\n

At its heart, health equity is about recognizing when our existing care and social systems fail\u00a0to<\/span>\u00a0reach and serve particular groups of people. It\u2019s a practice of asking, \u201cwho\u2019s missing from this picture?\u201d when we talk about health and well-being in our communities.<\/p>\n

For founder Sara Zia and the team at\u00a0Virginia Prison Birth Project (VAPBP)<\/a>, some of the people who are most invisible in healthcare spaces are pregnant people and those who\u2019ve recently given birth while incarcerated. \u201cThis is the most vulnerable segment of birthing people in our state,\u201d she says. \u201cReproductive health equity, it\u2019s not just the right\u00a0to<\/span>\u00a0have a baby or not, it\u2019s also the right\u00a0to<\/span>\u00a0have a baby in a safe way and\u00a0to<\/span>\u00a0feed your gestating fetus adequately and\u00a0to<\/span>\u00a0house yourself. It\u2019s so much larger, and it matters\u00a0to<\/span>\u00a0the work we\u00a0do<\/span>\u00a0because this population is hidden.\u201d<\/p>\n

Women are\u00a0the fastest growing proportion of people<\/a>\u00a0incarcerated in Virginia\u2019s jails and prisons. Estimates suggest that up\u00a0to<\/span>\u00a012,000 pregnant people<\/a>\u00a0are incarcerated nationally each year, although no agency keeps an official count. These pregnancies are at\u00a0high risk for complications<\/a>\u00a0because of limited access\u00a0to<\/span>\u00a0healthy nutrition or prenatal care among other factors. And in Virginia and nationally, Black residents are\u00a0overrepresented in incarcerated populations<\/a>, which compounds disparities in\u00a0maternal health and mortality for Black women<\/a>.<\/p>\n

Like all\u00a0do<\/span>ulas, VAPBP\u2019s\u00a0do<\/span>ulas are trained\u00a0to<\/span>\u00a0make sure that a pregnant person\u2019s voice is heard both before and during labor. Because they work with clients in prison, VAPBP\u00a0do<\/span>ulas\u00a0do<\/span>\u00a0this\u00a0advocacy and support work<\/a>\u00a0in both medical and carceral spaces. The organization began in the Charlottesville region, where Sara and her\u00a0do<\/span>ula team created cohorts for 12 weeks of education, nutrition, movement classes, and emotional support before clients give birth. In these sessions, they talk about\u00a0the anatomy and physiology of pregnancy, fetal development, prenatal care, childbirth while incarcerated, separation, lactation, parenting from inside, and reunification as a parent.<\/p>\n

As VAPBP expanded services into Henrico County Jail, they also worked with the sheriff\u00a0to<\/span>\u00a0bring in nourishing and familiar foods. Sara thinks that enjoying a meal\u00a0to<\/span>gether before class\u00a0contributes\u00a0to<\/span>\u00a0good nutrition and creates a \u201cconvivial\u201d space for the group. And the cohort model is key: \u201cEach cohort can form a pretty tight unit, especially at the prison because they live\u00a0to<\/span>gether and so it\u2019s huge when one of them [gives birth]. We know the others are thinking of them and are ready\u00a0to<\/span>\u00a0receive them when they come back—they know what it\u2019s like.\u201d<\/p>\n

When incarcerated pregnant people give birth, family members are not present, so the\u00a0do<\/span>ula functions as needed support and a familiar face. And they also help clients navigate a painful \u201cseparation visit,\u201d where they say goodbye\u00a0to<\/span>\u00a0their newborn before returning\u00a0to<\/span>\u00a0prison. When people who are incarcerated give birth, they usually have only a day or two with their newborn before the baby goes\u00a0to<\/span>\u00a0family members or other caregivers. \u201cIt\u2019s so hard\u00a0to<\/span>\u00a0prepare for that, but we help smooth things out and facilitate communication between the hospital, the prison, and the family and client in that space,\u201d Sara says.<\/p>\n

VAPBP\u00a0do<\/span>ulas will often make sure the parent has spent time with a soft blanket that goes home with the baby and that the parent gets\u00a0to<\/span>\u00a0keep the baby\u2019s first warm hat. This way, both have something that smells, comfortingly, like the other. Sara explains that \u201chelping\u00a0to<\/span>\u00a0establish that connection and advocating for clients\u00a0to<\/span>\u00a0be treated like human beings is worth all the struggles of working within this institutional framework. We aim\u00a0to<\/span>\u00a0nurture clients\u2019 connections with their babies however much we can within the constraints of giving birth in custody. Thinking of creative ways\u00a0to<\/span>\u00a0do<\/span>\u00a0this comes naturally\u00a0to<\/span>\u00a0our\u00a0do<\/span>ulas.\u201d<\/p>\n

One of the most necessary\u00a0things about VAPBP work is expanding efforts\u00a0to<\/span>\u00a0reach each individual client while also keeping in mind the systemic changes\u2014in sentencing and incarceration, in addiction and substance use treatment, in mental health care\u2014that could address these challenges at their roots. From Sara\u2019s perspective, \u201cThe biggest problems are community problems that will take a community solution.\u201d She cites\u00a0prison nursery initiatives<\/a>\u00a0that keep mothers and children\u00a0to<\/span>gether for longer as a step in the right direction: \u201cWe need facilities that house mothers and babies so that they have a chance\u00a0to<\/span>\u00a0catch their breath and we can come in and hear from them what their needs are.\u201d And, she says, \u201cIf we want\u00a0to<\/span>\u00a0have alternatives\u00a0to<\/span>\u00a0incarceration that are not punitive but more compassion-based, we have\u00a0to<\/span>\u00a0use evidence-based practices\u00a0to<\/span>\u00a0genuinely promote wellness and talk about the whole system of health equity.\u201d<\/p>\n

As part of its efforts\u00a0to<\/span>\u00a0address this range of community needs\u2014from maternal health\u00a0to<\/span>\u00a0substance use\u00a0to<\/span>\u00a0food access\u2014Health Equity Fund support will cover comprehensive\u00a0do<\/span>ula work for 25 new BAPBP clients in Richmond City. Support\u00a0includes weekly perinatal education, movement classes, and\u00a0do<\/span>ula advocacy during and after childbirth. And with each client VAPBP serves, and each time Sara shares this work, the picture of our health equity landscape becomes a little clearer.<\/p>\n
