This could include, but is not limited to, group practices, or private health organizations. Government entities are not eligible.<\/li>\n<li>Funds must only be used to support entities based in Virginia.<\/li>\n<li>Applicants must complete all required documentation to be eligible for the opportunity.<\/li>\n<li>Applicants must submit required documentation by the deadline provided.<\/li>\n<\/ul>\n<hr \/>\n<h2 style=\"margin-top: 20px;\">How to Apply<\/h2>\n<p>In order to be considered for selection, an applicant must submit completed applications no later than December 1 at 5 PM EST.\u202f Responses received after the submission due date and time will not be accepted.\u202f\u202fIncomplete responses \u2013 including responses missing required documents \u2013 will not be accepted.<\/p>\n<p>Applications must be submitted <a href=\"https:\/\/mvendor.cgieva.com\/Vendor\/public\/IVDetails.jsp?PageTitle=SO+Details&amp;rfp_id_lot=106566&amp;rfp_id_round=0\" target=\"_blank\" rel=\"noopener\">electronically through eVA<\/a>.
