Care Resource Coordination (CRC)

Care Resource Coordination is a program to assist those with human and social needs arising from or worsened by quarantine or isolation due to COVID-19. A Care Resource Coordinator (CRC) can connect those affected to local community organizations that may be able to assist with short-term needs, as well as ongoing needs following isolation or quarantine. Examples of local services provided by community partners include grocery assistance, utility bill payment assistance, benefits navigation for military/veterans, career development, legal services and more.

Doctors and laboratories are required by law to report positive COVID-19 test results to the local health district. If you test positive, you may be contacted by a contract tracer or case investigator, and they can refer you to the local care resource coordinator for assistance with care needs.
