We are delighted to announce the dates for Virginia’s Fourth Community Health Forum which will take place from August 26-27th, 2025. The format will be in-person sessions and will be hosted at the Beacon Theater, 401 N Main Street, Hopewell, VA 23860.
The theme of 2025 CHF arises from the common priority areas identified in Virginia CHA/CHIPs, aiming to unite public health and healthcare professionals to achieve the desired outcomes of various organizations across multiple sectors. The overall mission of the forum is to elevate Virginia CHA/CHIPs as a cornerstone for the health, prosperity, and resilience of our communities. Through collaboration, education, and actionable steps, we will ensure that every Virginian understands and integrates these common priority area strategies into their daily practices, helping to create a sustainable, thriving Commonwealth for all and ultimately making Virginia the healthiest state in the nation. In addition, the 2025 CHF will offer an opportunity to collectively educate and revitalize our public health and healthcare workforce, with an intent to create equitable access to healthcare and resources through panel discussions, keynote speakers, and informational sessions.
Location & Accommodations
The conference will take place at The Beacon Theater, 401 N Main Street, Hopewell, VA 23860. There are some rooms available under a block rate of $110 USD per night on a first come-first serve basis.

Abstract Submission
We are now accepting abstracts for poster presentations for the Networking and Poster Session, August 26, 2025, from 4:30pm - 5:30pm. The deadline for abstract submission is August 10, 2025. You will be notified if your abstract is accepted for poster presentation by August 13, 2025.
This year's Community Health Forum's theme is bringing public health and healthcare professionals together to achieve outcomes desired by organizations across multiple sectors. Abstract submitters are encouraged to submit abstracts on the forum theme of current and emerging public health issues that you have worked on in the past or something you are currently working on in poster format.

Award Nominations
The Virginia Department of Health’s Center for Community Health Improvement and the Community Health Improvement Epidemiology Program will host the Office of Family Health Services 2025 Community Health Forum on August 26-27 in Hopewell, VA. This year’s forum, inspired by CHA/CHIPs throughout the Commonwealth of Virginia, aims to unite public health and healthcare professionals across sectors to achieve shared health outcomes. The event will feature keynote speakers, breakout sessions, and engaging activities focused on collaboration, innovation, and equity in community health.
In addition, the forum will present the Virginia Community Health Improvement Awards, recognizing outstanding contributions in categories including Population Health Manager of the Year, Community Health Epidemiologist of the Year, Emerging Community Health Professional, and Community Health Improvement Partner of the Year. These awards honor leadership, innovation, collaboration, and measurable impact in advancing health equity and improving community outcomes. Nominations are open through August 10, 2025.
