Written Respiratory Protection Plan Template
This document was prepared by the Virginia Department of Health to serve as a template for medical offices/organizations where respirators are used to develop and implement their written respiratory protection program as required by the OSHA Respiratory Protection Standard (29 CFR 1910.134). Throughout the template, there are technical and editorial notes that do not need to be retained but are meant to guide individualizing the plan. It includes the following appendices: 1) list of employees using respirators; 2) Medical questionnaire; 3) Work factors for respirator use; 4) Medical recommendation for respirator use; 5) Respirator Fit-test record; 6) Information for employees using respirators when not required under the OSHA standard; 7) References.
