Title: Public Health Nurse Senior, Pre-assessment screenings 

Location: Henrico East 

Where does your public health work take you?  

I complete home visits throughout the community for the CCC+ Waiver. The waiver can provide long-term care services covered by Medicaid, which includes things like nursing home placement or a home health aide. When a family requests help with these services, a nurse and a social worker will go to the client’s home to conduct a screening, which is like a thorough nursing assessment, to see if they meet the waiver’s criteria. The RN and social worker assess if they need help with activities of daily living like showering, walking, or using the toilet. We look for ongoing medical/nursing needs and risks for hospitalization or nursing facility placement: Have they been hospitalized recently? Do they have dementia and they’re wandering? Conducting these assessments helps some of our most vulnerable community members get the care they need.

The ICU and critical care.  


Where do you get your public health inspiration? 
 

A professor of mine from nursing school still inspires me today. She had so much knowledge about nursing and healthcare, and her example reminds me to keep learning and growing.
