Our population health team is available to help with the following types of outreach:

Community Health Assessment
The community health assessment (CHA) is required to be conducted every 5 years at the health department level; non-profit hospitals are required by the IRS code to complete a community health needs assessment (CHNA) every 3 years. Often times, these assessments are completed in partnership and therefore our CHA for the Southside Health District is usually completed every 3 years.
The Southside Community Health Assessment was published by the Southside Health District with data from all three localities: Halifax County, Mecklenburg County, and Brunswick County.
The Community Health Improvement Plan (CHIP) is the follow-up actions on the community member identified priorities led by work with community partners and volunteers. We are currently searching for organizations and individuals who would like to assist with this project. Southside’s current priority areas are Access to Healthcare Services; Aging & Eldercare; Chronic Disease.
