Diabetes Education Program

If you live in Franklin, Southampton County or Suffolk and need help with managing your diabetes call 757-562-2169 for an appointment today.  We offer free education on:

  • Diabetic screening
  • Medications, including insulin
  • Diabetic complications
  • Dietary counseling

Medication Assistance

If you cannot afford your prescription medication or nutritional supplement, help may be available pending eligibility:

  • You must be a resident of Franklin or Southampton County OR have a doctor in Franklin or Southampton County
  • You do not have prescription drug insurance or are paying high co-pays
  • You have Medicare part D and are paying full price or high co-pays for your medications
  • Income-based

Nurse-Family Partnership

The Nurse-Family Partnership is an evidence-based community health program that helps transform the lives of vulnerable mothers pregnant who are 28 weeks pregnant or less with their first child. The program is a voluntary prevention program that provides registered nurse home visitation services to low-income, first-time mothers.

Safety Seat Program

The Safety Seat Program is a statewide child safety seat distribution and education program and is offered free of charge to eligible families.

Services are provided to all for the purpose of prevention, detection, and follow-up of contagious diseases.

Health Education 

The district is active in health education activities including community screenings, referral and counseling.
