Child Safety Seat Program

CAR SEAT PROGRAM – LISSDEP
The Safety Seat Program is a statewide child safety seat distribution and education program and is offered free of charge to eligible families.

To qualify for a free child safety seat or booster, applicants must be:
• Eligible for WIC, Medicaid or FAMIS
• A resident of Virginia – Western Tidewater Health District (Suffolk, Isle of Wight
County, Franklin or Southampton County)
• A custodial parent, legal guardian, or foster parent of a child who is seven years old or
younger, or at least in the last trimester of pregnancy and whose child fits within the
program’s safety seat manufacturer’s guidelines
• Attend a safety seat class in its entirety (NEW ONLINE TRAINING)
• Sign a waiver of liability release form

• Application must be completed in its entirety – signed and dated (for multiple car
seat requests, a separate application for each child receiving the car seat must be
filled out)
• Attach the following information to be considered
• Picture ID (ID Card or Driver’s License)
• WIC/Medicaid Card (Mothers ONLY)
• If you are a foster parent or legal guardian, you must show legal documents
