Long Term Services and Supports Screenings (Pre-admission Screenings) are required for any person requesting care in a Medicaid certified facility, or for in-home care who are eligible and approved for Medicaid. A detailed assessment is performed by a Public Health Nurse and a Family Service Worker, to determine need and appropriate setting for care. Based on the assessment and recommendations for care, the individual and their family are given information to contact services available to them.
