Current Community Health Assessments and Improvement Plans:

2025-2029 Hanover County Community Health Improvement Plan

Hanover County has started developing the 2025-2029 Community Health Improvement Plan (CHIP). The CHIP will outline how the community plans to address the top three health priorities: 1) Healthy Eating and Food Insecurity; 2) Mental Health; and 3) Affordable, Safe, and Available Housing. Getting input from the community is important for making a community health improvement plan (CHIP). Your ideas help make sure the plan works well for everyone, represents the community’s needs, and makes Hanover County healthier. Do you have a great idea on how to address the health priorities? Share it with health department staff by emailing CHDWellness@vdh.virginia.gov or calling 804-365-3240. The CHIP has a planned release of March/April 2025.

2024 Goochland Community Health Assessment and Community Health Improvement Plan

The 2024 Goochland County Community Health Assessment is set to be published in February 2025. Additionally, Goochland County has started developing the 2026-2030 Community Health Improvement Plan (CHIP). The CHIP will outline how the community plans to address the top three health priorities: 1) Aging Population Support and Services; 2) Mental Health; and 3) Affordable, Safe, and Available Housing. The CHIP has a planned release of June/July 2025.
