The Eastern Shore Health District’s Population Health staff works with many community partners to improve the health of people in our area. The team focuses on understanding the issues that affect health on the Eastern Shore and works to improve health by involving residents and partners.
The team includes Community Health Workers who help people in the community use the health care system and connect them with local resources.
