The Danville/Pittsylvania County Healthy Hearts Learning Collaborative (Pitt-Dan LC) is owned and managed by community members. The Virginia Department of Health, as a core partner, provides ongoing support including state and local public health collaboration, chronic disease prevention program training and resources, chronic disease data, leadership and advocacy training, funding opportunities, survey and newsletter design, and webpage maintenance.
Mission: The Pitt-Dan Healthy Hearts Learning Collaborative empowers people to take charge of their heart health through awareness, advocacy, and fair access to prevention education and care - because every heartbeat matters.
Vision: The Pitt-Dan Healthy Hearts Learning Collaborative unites individuals, families, and professionals to promote heart health, healing, and hope through education, empowerment, and shared compassion. Together, we strive to prevent heart disease, support recovery, and build stronger, healthier communities.
Pitt-Dan LC Goals:
- Encourage participants and community members to check their blood pressure five times per week at the same time each day.
- Increase number of community members who know how to properly take their blood pressure.
- Educate community members on proper nutrition to maintain a healthy heart.
- Provide smoking cessation assistance to increase heart health.
- Increase physical activity of the LC and community members.
