Three Rivers Health District Maternal and Child Health Services

In addition to Three Rivers Health District's family planning and WIC (Women, Infants, and Children) programs, the health district also provides care navigation for young and expecting families through our Community Health Worker and Resource Mothers programs.  More information about these services can be found in the fliers below, and clients interested in receiving services can complete the Three Rivers Health District Maternal Health Support Intake Form.
