New RFA Opportunities NOW OPEN for Rural Health Transformation in Virginia

The Virginia Department of Health (VDH) has opened two Requests for Applications (RFAs) under Virginia’s Rural Health Transformation Program (RHTP): Community Paramedicine and Innovative Maternal Care. These funding opportunities will support innovative, community-driven approaches to expand access to care, strengthen local and regional health systems, and improve health outcomes for rural Virginians.

Virginia Department of Health Leading Two Open Opportunities

  • Community Paramedicine: VDH is seeking community-driven Community Paramedicine and Mobile Integrated Healthcare projects that expand access to care and preserve emergency response EMS resources in rural Virginia. Projects may include treat-in-place and alternative care delivery models, chronic disease management, care transitions and readmission prevention, preventive services, behavioral health and substance use supports, care coordination and patient navigation, aging-in-place initiatives, and other innovative approaches that connect rural residents with the right care at the right time.
    • Funding Available: Approximately $3.7 million, with awards of up to $400,000 per project.
  • Innovative Maternal Care: VDH is seeking innovative, community-driven projects that improve access to prenatal, postpartum, and other maternal healthcare services in rural Virginia. Projects may include maternal health access hubs, innovative prenatal and postpartum care models, mobile or technology-enabled care, behavioral health and substance use supports, transportation and access solutions, community health workers and doulas, care coordination, workforce development, and regional partnerships that strengthen rural maternal health systems.
    • Funding Available: Approximately $11.6 million, with awards ranging from $50,000 to $1 million per project.

eVA Scheduled Blackout Notice

Please be advised that eVA will undergo a scheduled blackout period beginning September 10 at 7:00 PM ET and concluding September 14 at 7:00 AM ET. During this time, eVA will be inaccessible. Application materials and related documents for the Community Paramedicine and Innovative Maternal Care RFAs will be available on September 8, 2026, 5:00 PM ET and throughout the blackout period.

 

Applicant Webinars & Q&A

VDH will host informational webinars on September 16, 2026, for prospective applicants. Each webinar will provide an overview of the respective funding opportunity, application requirements, and submission process, followed by a Q&A session.

Prospective applicants may also submit questions during the RFA Q&A period. Questions must be submitted no later than September 25, 2026, at 5:00 PM ET. Responses to questions received during the Q&A period will be compiled and posted as an addendum in eVA following the close of the Q&A period.

Virginia’s RHTP was established under Section 71401 of Public Law 119-21 and is administered by the Centers for Medicare & Medicaid Services. CMS awarded the Commonwealth a cooperative agreement designating the Virginia Department of Medical Assistance Services (DMAS) as the primary recipient. VDH is a subrecipient of DMAS and administers the Community Paramedicine and Innovative Maternal Care sub-initiatives.
