Frederick Co / Winchester City

Nutritionist Associate

The Nutritionist Associate independently determines nutrition and income eligibility of applicants/clients within program guidelines. Assesses nutritional status/eligibility for program services by: dietary recall/food frequency history, assessing growth patterns, weight patterns, biochemical values and medical history, and independently prescribes appropriate food packages. Independently sets dietary goals based on the assessment. Provides patient counseling specific to the individual’s nutritional needs. Makes referrals for high risk counseling with Nutritionist when appropriate.

Shenandoah County

Public Health Nurse

Independently provides professional nursing care to patients in clinics, homes and schools and other community or congregate care settings. Performs nursing assessments, may perform lab tests, take measurements, develop and implement care plans, including the administration of medication and case management. Participates in public health emergencies and community health assessments. Actions performed at this level require licensure as a RN by state code.

Nutritionist

Provides nutrition services to clients in a variety of settings within a district. Characteristic activities: conducts patient nutritional assessments and evaluations in the clinic, school and community settings; develops and monitors nutritional care plans and food packages. Certifies clients for participation in available public health programs; schedules, provides and documents basic and secondary/high risk nutrition education; makes referrals to programs such as Medicaid, AFDC, and SNAP benefits.
