The Community Health Worker (CHW) will be responsible for community outreach, health resources navigation, and community advocacy within specific localities within the Lord Fairfax Health District, specifically related to Community Health Assessments, health education, and COVID-19 vaccination. This position will also engage local communities to promote health department services and provide health education in formal and informal settings. Key responsibilities include assisting with community outreach and engagement of health education, provide health resources navigation for community members, and community health advocacy for local health departments and their efforts for community improvement.

Breastfeeding Peer Consultant
The Lord Fairfax Health District seeks to fill a part-time wage Breastfeeding Peer Counselor position to work with clients at the home base being Shenendoah County but also traveling to Page and Warren County. The Breastfeeding Peer Counselor promotes breastfeeding among WIC participants, health care workers and the community. Attends local, regional, or state breastfeeding promotions and supports training. Operates within the BFPC scope of practice as defined in the WIC Breastfeeding Curriculum.

Nutritionist
The Lord Fairfax Health District is seeking an enthusiastic team member to join the Women, Infants and Children (WIC) program serving all localities within the district with a primary focus in the Warren County and Page County Health Department. This position is a full-time salaried Nutritionist position serving the WIC participants.
Provides nutrition services to clients in a variety of settings within a district. Characteristic activities: conducts patient nutritional assessments and evaluations in the clinic, school and community settings; develops and monitors nutritional care plans and food packages. Certifies clients for participation in available public health programs; schedules, provides and documents basic and secondary/high risk nutrition
education; makes referrals to programs such as Medicaid, AFDC, and SNAP benefits. May perform other related administrative tasks.
Nutritionist Associate
The Nutritionist Associate independently determines nutrition and income eligibility of applicants/clients within program guidelines. Assesses nutritional status/eligibility for program services by dietary recall/food frequency history, assessing growth patterns, weight patterns, biochemical values and medical history, and independently prescribes appropriate food packages. Independently sets dietary goals based on the assessment. Provides patient counseling specific to the individual’s nutritional needs. Makes referrals for high-risk counseling with Nutritionist when appropriate.
