Chesterfield Health District will host vaccine clinics for Chesterfield, Powhatan, and Colonial Heights students entering 7th and 12th grades at the Chesterfield/Colonial Heights Health Department at 9501 Lucy Corr Circle, Chesterfield, VA 23832 in the Multi-Purpose Room.
We are unable to take walk-ins and parents must pre-register using the links below. We have limited appointment slots.
- Parents can check their student's records before enrolling them.
- Read the vaccine information sheets at the links under each vaccine below:
- In the middle schools we will administer the Tdap, Meningococcal ACWY, and HPV vaccines.
(Vacuna Tdap, meningocócica ACWY, Vacuna contra el VPH) - In the high schools we will administer the meningococcal ACWY vaccine. (meningocócica ACWY)
- If a student should need other immunizations, we will share additional resources.
- In the middle schools we will administer the Tdap, Meningococcal ACWY, and HPV vaccines.
- Parents should register their students online (at the link for each school below) to receive these immunizations.
♦ Enter the student's insurance information if the student is covered by insurance. Upload a photo of the front and back of the card where indicated.
♦ Check the boxes for all immunizations that are needed.
♦ If you have a problem with registration, call 804-748-1750 for assistance.
♦ Please register ahead of time for these clinics to allow us time to evaluate the records and insurance. Internet access in this area is not reliable so we need as much notice as possible. - Registration deadlines:
- Clinic on Friday, July 24 with registration ending Monday, July 20 at 10:30 p.m.
- Clinic on Monday, July 27 with registration ending Monday, July 20 at 10:30 p.m.
