Background
COVID-19, caused by the SARS-CoV-2 virus, emerged in late 2019 and has since become a global pandemic. It primarily spreads through respiratory droplets and can lead to a range of symptoms, from mild to severe. Efforts to control the virus include vaccination, social distancing, and hygiene practices. Data & Reporting

- COVID-19 Nursing Home Data (CMS) – Resident and staff COVID-19 vaccination data are available by nursing home and state
National Healthcare Safety Network (NHSN)
Resources related to reporting COVID-19 data to NHSN are available on the CDC website, including protocols, data collection forms, and analysis resources. For more information about reporting COVID-19 vaccination data, see CDC’s frequently asked questions.
- Dialysis facilities: COVID-19 vaccination reporting requirements for Virginia dialysis facilities are established by Quality Insights.
- Facilities that are part of the CMS Inpatient Quality Reporting Program, Inpatient Psychiatric Facility Quality Reporting Program, Inpatient Rehabilitation Facility Quality Reporting Program, or Long-Term Acute Care Quality Reporting Program: These facilities are required to submit COVID-19 vaccination data via the Weekly COVID-19 Vaccination Module for at least one week per month to fulfill CMS reporting requirements.
- Long-term care facilities:
- CMS-certified long-term care facilities are required to report various COVID-19 case and/or vaccination data through the National Healthcare Safety Network’s Respiratory Pathogens and Vaccination Module. There is protocol for both facility residents and healthcare personnel.
- LTCFs also have the option to report COVID-19 test results provided by a POC test device through the NHSN Point-of-Care (POC) Test Reporting Tool. Important: the reporting of POC test result data in this tool does not take the place of answering test-related questions in the Respiratory Pathogens and Vaccination Module.
- Skilled nursing homes certified by CMS may report the POC test results to either the VDH portal or to NHSN. Facilities only need to report to one system, as results from NHSN are reported to appropriate state and local health departments using standard electronic laboratory messages.
Virginia Communicable Disease & Outbreak Reporting Requirements
- Per the Regulations for Disease Reporting and Control, the following is reportable:
- By physicians and directors of medical facilities: Report not only when a person who is infected with or who is suspected of having COVID-19 is treated or examined, but also if the person is hospitalized or admitted into an intensive care unit.
- The report must be sent electronically within three days of the suspicion or confirmation of disease using either VDH’s online Confidential Morbidity Report Portal or Electronic Case Reporting.
- By directors of laboratories and other entities that hold a Clinical Laboratory Improvement Amendments Certificate of Waiver (e.g., pharmacists): Coronavirus disease 2019 (COVID-19 or SARS-CoV-2) – Positive SARS-CoV-2 test results only
- The report must be submitted electronically within three days of identification using either VDH’s available portal for laboratory reporting (VDH’s COVID-19 Point of Care (POC) Test Reporting Portal) or Electronic Laboratory Reporting.
- By facilities or programs licensed or operated by any agency of the Commonwealth, schools, child-care centers, and summer camps: Any suspected or confirmed outbreak
- By physicians and directors of medical facilities: Report not only when a person who is infected with or who is suspected of having COVID-19 is treated or examined, but also if the person is hospitalized or admitted into an intensive care unit.
